The Encore Dental® insurance plan, underwritten by Stonebridge Life
Insurance Company, can significantly cut your dental bills, no matter what dentist
you see. The benefit levels shown in the above chart indicate what the plan will
pay when you visit one of the plan's in-network dentists. If you decide to see a
dentist who is not part of the plan’s network, you also pay for any amount
above the “eligible expense” (the smallest of either the medium charge,
or 85% of the usual and customary charge for the area where the dental service is
received). There is a 12-month waiting period to receive
benefits for major dental services.
